Any permission slip needed for Wiley trips, events, or excursions.

  • This permission slip covers your student for the entire year.  Any trip or activity that occurs off of Wiley Union Church grounds requires a permission slip be on file for each attendee.

  • TACTICAL TAG LASER TAG | Needed for July 30th, 2024

    This online waiver must be filled out before you can participate in our laser tag event at Tactical-Tag in Yakima, Washington.  Students must also have a Wiley Union Church Permission Slip on file for the current year.

  • HIGH STEPPE CLIMBING GYM ONLINE WAIVER | Needed for August 13th, 2024

    This online waiver permits our students to learn to rock climb at High Steppe Climbing Gym right here in Yakima, WA.  Click the blue square and then watch the video before filling out the waiver online.  Students must also have a Wiley Union Church Permission Slip on file for the current year.

  • This online waiver must be filled out before you can participate in our event at Quake in Kennewick, Washington.  Students must also have a Wiley Union Church Permission Slip on file for the current year.

  • This permission slip grants your student permission to play paintball with us during Camp 2023 at Operation Paintball, in Graham, Washington.  Students must also have a Wiley Union Church Permission Slip on file for the current year.
